Liability Waiver

I understand that participation in Taekwondo training involves physical activity and carries an inherent risk of injury. By submitting this booking request, I voluntarily agree to participate and hereby release Traditional Taekwondo Batavia and instructor Michael Peintinger from any and all liability, claims, demands, and causes of action arising out of or related to any loss, damage, or injury that may be sustained during training sessions.

I confirm that I am in adequate physical health to participate, or that I have consulted a physician prior to beginning training. I understand that the dojo address will be provided upon booking confirmation and must not be shared publicly.

Lesson Fee
$50 per session

Payment is due at the start of each lesson. We accept cash or personal check payable to Michael Peintinger. No payment is required to submit this booking request.
